Description
When a chapter is created , the name of the chapter by default appears BOLD-ed in the body part of edited chapter.
If you leave it unchanged , it may not cause problems. Chapters will have proper numbering, etc.
If you Change the Bolded part to something else - you will end up having two chapter names:
a. The one in the Table of Content: http://booktype-dev.sourcefabric.org/euro2012/_edit/#
b. and the one you just changed.
Expected behaviour:
There should be centralized editing for the chapter names.
All the changes should be synchronised

unless shown the opposite, I see the downside of current not-syncronised state is that the non-unique chapter names are creating mess in certain scenario (described related issues in the tickets BK-275, BK-276, BK-277)

i dont think it has to do with non unique names. it has to do with not using an h1 for the chapter title inside the content. Please also try the tests with h1 and moving around chapters in toc.
